To request a change to this list, including additions or edits, send an e-mail, preferably bearing a company logo, if you have one. Otherwise please expect a phone call verifying the request. You can send the email to Russ Meyer (rmeyer@crcpd.org) directly.
For edits, the least confusing way to word the request is to copy the entry for your facility from the list. Replace those words or numbers you wish to change and send the e-mail request with the new entry to the e-mail address above. For lists with multiple sections, please also enter the sections in which you wish your information entered or changed. The Office of the Executive Director will make the changes on the latest posted list on the website as soon as possible.
